Add coffee favicon to all fuzzyworld sites

- Add favicon.ico link to fuzzyworld.net index.html
- Add favicon.ico link to all pages generated by gen.py (code.fuzzyworld.net and apps.fuzzyworld.net)
- Copy favicon.ico to code.fuzzyworld.net and apps.fuzzyworld.net in install.sh

This ensures the coffee favicon appears on all fuzzyworld sites, not just fuzzyworld.net.
