Deploy to apps.fuzzyworld.net with fuzzyworld-tools

Adds the fuzzyworld package.json section, a deploy script (fw deploy) and the fuzzyworld-tools dependency, and points gitlab.io / gitlab.com links at apps.fuzzyworld.net and code.fuzzyworld.net.
